Arogyam Allergy – Not providing the medical treatment proof of the patient

Sir, My name is Paramjit Singh and I live in Jalandhar, Punjab. At the end of the year 2012, my sister Sukhwinder Kaur age 34, faced some health issues (Depression/Migraine) and started taking Ayurvedic medicine from Arogyam Ayurvedic in Model Town, Jalandhar. The Doctor gave her a card and wrote down the serial no. 1830. She stared using medicine and used almost 6 months but never asked for medicine slip/receipt neither the doctor gave her because a patient doesn’t care about or understand the value of these kinds of proofs for future use and just believe in the doctors and rely upon their medicines that were given. After taking medicine for 6 months, we noticed significant improvement in her heath but decided to take medicine form somewhere else. In December 2014, my sister went abroad and now she urgently required some proofs of her medicine that she had been taking form Arogyam Ayurvedic clinic. For this purpose, we visited Arogyam on 28, 29th December but the managing Director, Dr. Satnam Singh was not present at the time. We visited again on 30th December and a lady doctor asked to SUBMIT OUR SLIP on the counter and said that the Doctor will come tomorrow and only he could find a solution for it. So, without any doubt, I submitted the card/slip. On next day, finally we met Dr. Satnam Singh and he refused to give any proof for the patient by telling that he didn’t find any record of the patient with this serial number because this is an “old” slip and they do not keep records of the old patients. Further, on my question about keeping records in the computers, he “proudly” said that THEY DON’T HAVE COMPUTERS. It was clear that he don’t want to give any proof. So, I asked for the slip, the only proof that I Submitted one day ago and then the staff members tried to show that they have MISPLACED it somewhere. But in few minutes they provide us a new card and wrote down the same serial number and name of the patient as it was on the old one. I have attached the pictures of the card with this complaint. I don’t want to submit this complaint in written format to any other department. So, I request to the head office of Arogyam to resolve our issue and provide us a proof of medicine/treatment that was given to my sister because it is urgently required in a new country for further treatments of her.
